Introduction: HIV disease was transformed from a fatal condition to one with metabolic complications. In Mexico City, the associated factors for prediabetes in this population are unknown; investigating it is relevant to improve their quality of life. Objective: To determine the risk association factors for prediabetes in people living with HIV. Material and methods: Cross-analytical, retrospective study. Records of patients living with HIV were reviewed, exposure factors and fasting glucose concentration were recorded. Patients from 18 to 65 years of age were included, on co-formulated antiretroviral treatment, without adjustment of antiretroviral treatment in the last two years, with a BMI of 18.5-40 kg/m2. Results: 148 patients were included, 68 presented prediabetes. The factors with risk association that were identified are: age over 60 years (OR 9.48, 95% CI 1.68-40.13), treatment with Efavirenz/Tenofovir/Emtricitabine (OR 9.28, 95% CI 2.55-33.74) and treatment time antiretroviral older than 12 months (OR 2.53, 95% CI .912-7.041). Conclusion: The prevalence of prediabetes in people living with HIV is 46%. The main associated factor was the consumption of Atripla. This study has clinical relevance since it will allow the implementation of prevention, diagnosis and treatment strategies for prediabetes in order to reduce associated morbidity and mortality.


Introducción: la enfermedad por VIH ha pasado de ser un padecimiento mortal a uno con complicaciones metabólicas. En la Ciudad de México se desconocen los factores asociados para prediabetes en esta población, investigarlo es relevante para mejorar su calidad de vida. Objetivo: determinar los factores con asociación de riesgo para prediabetes en personas que viven con VIH. Material y métodos: estudio transversal-analítico, retrospectivo. Se revisaron expedientes de pacientes que viven con VIH, registrándose los factores de exposición y la concentración de glucosa en ayuno. Se incluyeron pacientes de 18 a 65 años, en tratamiento antirretroviral coformulado, sin ajuste de tratamiento antirretroviral en los últimos dos años, con IMC de 18.5-40 kg/m2. Resultados: se incluyeron 148 pacientes, 68 presentaron prediabetes. Los factores con asociación de riesgo que se identificaron son: edad mayor de 60 años (OR: 9.48, IC95%: 1.68-40.13), tratamiento con Efavirenz/Tenofovir/Emtricitabina (OR: 9.28, IC95%: 2.55-33.74) y tiempo de tratamiento antirretroviral mayor de 12 meses (OR: 2.53, IC95%: 0.912-7.041). Conclusión: la prevalencia de prediabetes en personas que viven con VIH es del 46%. El principal factor asociado fue el consumo de Atripla. Este estudio tiene relevancia clínica ya que permitirá implementar estrategias de prevención, diagnóstico y tratamiento de prediabetes con la finalidad de reducir la morbimortalidad asociada.

BACKGROUND: Men who have sex with men (MSM) and transgender women (TGW) are disproportionately affected by HIV, with much higher incidence and prevalence rates than in the general population in different countries. There are several barriers to testing among MSM and TGW, such as low risk perception, anticipation of HIV-related stigma, discrimination of sexual orientation, in addition to difficulties related to care and access to health services. Therefore, analyzing the available evidence of the effectiveness of strategies for scaling up HIV testing among key populations is essential to point out potential knowledge gaps which may need to be addressed and develop public health policies to promote testing and early diagnosis of HIV infection. METHODS: An integrative review was carried out to evaluate strategies for scaling up HIV testing in these populations. Search strategy was performed on eight electronic databases, without language restriction. We included clinical trials, quasi-experimental studies, and non-randomized studies. Study selection and data extraction were both performed independently by pairs and disagreements were solved by a third revisor. The screening of the studies was carried out through the selection of titles/abstracts and the reading of the full texts of the pre-selected studies based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Data extraction was performed using a structured form. RESULTS: Thirty-seven publications referring to 35 studies were included, mostly being carried out in the United States of America and Australia. No studies were found evaluating disaggregated data on TGW. The studies were grouped into four types of intervention strategies: self-test distribution system (n = 10), organization of health services (n = 9), peer education (n = 6), and social marketing campaign (n = 10). Strategies that focused on the first three groups, combined or not, were more effective in increasing HIV testing among MSM. CONCLUSIONS: Considering the diversity of interventions and the methodological heterogeneity of the included studies, strategies especially involving self-test distribution systems, associated with new information and communication technologies, should be evaluated in different communities and social contexts. Research evaluating specific studies on TGW population is still needed.

Introduction: Lipodystrophy is an important complication of HIV and has different clinical manifestations, such as lipoatrophy of the face, buttocks, and limbs and accumulation of fat in the abdominal and cervical regions. Lipodystrophy has aesthetic and psychosocial consequences, stigmatizing and affecting patients' quality of life. The objective is to evaluate the epidemiology and treatments performed in patients treated at the HIV-related Lipodystrophy Outpatient Clinic at Hospital das Clínicas da Faculdade de Medicina de Botucatu. Method: The study was conducted retrospectively, with port analysis of patients treated between June 2012 and December 2019, at Hospital das Clínicas da Faculdade de Medicina de Botucatu, in Botucatu, SP, Brazil. Results: The medical records of 153 individuals were analyzed, 79 male and 74 female patients. The mean age was 45.6 years. The referrals came from 48 cities in four states. Caucasian patients accounted for 74.5% of the consultations. The complaint of facial lipodystrophy was reported by 52.9% of the patients. The most common invasive procedure was facial filling with polymethylmethacrylate (PMMA) in 62 patients. Gluteal implants were the most common surgery on six occasions. Conclusion: The data found show a higher proportion of female patients with complaints of lipodystrophy when compared to general data of patients with HIV. The white race was predominant, and the main complaint of lipodystrophy was facial atrophy. Facial filling with PMMA was the most common procedure.


Introdução: A lipodistrofia é uma importante complicação do HIV e apresenta diferentes manifestações clínicas, como lipoatrofia de face, glúteos e membros, e acúmulo de gordura em região abdominal e cervical. A lipodistrofia apresenta consequências estéticas e psicossociais, que são estigmatizantes e afetam a qualidade de vida dos pacientes. O objetivo é avaliar a epidemiologia e os tratamentos realizados nos pacientes atendidos no Ambulatório de Lipodistrofia relacionada ao HIV do Hospital das Clínicas da Faculdade de Medicina de Botucatu. Método: O estudo foi realizado de maneira retrospectiva, com análise de portuário dos pacientes atendidos entre junho de 2012 e dezembro de 2019, no Hospital das Clínicas da Faculdade de Medicina de Botucatu, em Botucatu, SP, Brasil. Resultados: Os prontuários de 153 indivíduos foram analisados, sendo 79 pacientes do sexo masculino e 74 do sexo feminino. A média de idade foi 45,6 anos. Os encaminhamentos tiveram origem de 48 cidades, de quatro estados. Pacientes da raça branca totalizaram 74,5% dos atendimentos. A queixa de lipodistrofia de face foi referida por 52,9% dos pacientes. O procedimento invasivo mais realizado foi o preenchimento facial com polimetilmetacrilato (PMMA), em 62 pacientes. A inclusão de implantes glúteos foi a cirurgia mais realizada, em seis ocasiões. Conclusão: Os dados encontrados mostram maior proporção de pacientes do sexo feminino com queixa de lipodistrofia, quando comparados a dados gerais de pacientes com HIV. A raça branca foi predominante e a principal queixa de lipodistrofia foi a atrofia facial. O preenchimento facial com PMMA foi o procedimento mais realizado

BACKGROUND: Transgender women sex workers (TGW-SW) are disproportionally affected by HIV and have reduced access to testing. Moreover, information regarding their behaviours and health needs is scarce. METHODS: A behavioural survey and a targeted testing programme in prostitution sites were conducted in Milan and Monza areas. The non-profit organisation 'ALA Milano Onlus' and 'San Gerardo' Hospital (Monza) implemented a mobile HIV testing unit involving a TGW peer educator, four physicians, a counsellor, a psychologist and a cultural mediator. All TGW-SW were offered anonymous HIV and hepatitis C virus (HCV) oral testing and asked to fill a questionnaire on sexual habits, drug abuse, and knowledge and attitudes towards HIV and STDs. RESULTS: Between May and July 2017, 130 TGW-SW, predominantly migrants, were contacted during 15 street visits; among them, 78 (60%) were interviewed. HIV and HCV testing were accepted by 53 (42%) and 67 (52%) TGW-SW, respectively. Twenty-five (19.8%) subjects who reported already established HIV infection were not retested. Seven patients received a new diagnosis of HIV, while nobody tested positive for HCV. Overall, HIV prevalence was 13.2% (25% including those with already known HIV infection). Recent arrival in Italy and young age were associated with risk of undiagnosed HIV infection. Inconsistent condom use was commonly reported during commercial sex (27%) and with non-commercial partners (64%). Alcohol and cocaine abuse were common problems which facilitated risky behaviours. CONCLUSIONS: Oral rapid HIV and HCV testing for TGW-SW in outreach settings were feasible and acceptable and led to a considerable number of new diagnoses. Interventions tailored to TGW-SW, focused on HIV prevention, testing and engagement in care, are fundamental.

RESUMO Objetivo estimar a prevalência e fatores associados à infecção pelo Human Immunodeficiency Virus (HIV) em jovens. Métodos estudo transversal, realizado em dois Centros de Testagem e Aconselhamento, com 279 jovens entre 15 e 24 anos. Realizou-se teste anti-HIV e aplicou-se instrumento estruturado. Empregaram-se análise bivariada e regressão logística com correção de Bonferroni para investigar a associação entre as variáveis e o desfecho sorológico. Resultados a prevalência de HIV entre os jovens foi de 3,9% (Intervalo de confiança-IC95%: 1,0-10,1). A associação com o desfecho HIV mostrou-se estatisticamente significante entre jovens do sexo masculino (p=0,001), homossexuais (p<0,001), exposição sexual como motivo do teste (p=0,034), percepção de alto de risco de adquirir HIV (p=0,002) e prática de sexo anal (p=0,004). Jovens homossexuais (Odds ratio-OR=13,46; IC95%:1,14-15,84) e com percepção de alto risco de adquirir o HIV (OR=18,11; IC95%: 2,28-143,69) tiveram maiores chances de apresentarem resultado positivo para HIV. Conclusão foi possível identificar a prevalência e fatores associados ao HIV em jovens. Contribuições para a prática: as informações geradas podem disponibilizar evidências aos formuladores de políticas, gestores e profissionais de saúde, e subsidiar a elaboração de estratégias que contemplem o aperfeiçoamento das ações de prevenção, coordenadas e ajustadas aos contextos comportamentais nos cenários estudados.


ABSTRACT Objective to estimate the prevalence and associated factors with Human Immunodeficiency Virus (HIV) infection in youths. Methods cross-sectional study in two Centers for Testing and Counseling, with 279 youths from 15 to 24 years old. An anti-HIV test and a structured instrument were applied. We used bivariate analysis and logistic regression, with Bonferroni corrections, to investigate the association between the variables and the outcome. Results the prevalence of HIV in the youth analyzed was 3.9% (Confidence interval - CI95%: 1.0-10.1). The association with the outcome of HIV was statistically significant in male youth (p=0.001), homosexuals (p<0.001), those who tested due to sexual exposure (p=0.034), perception of a high risk of acquiring HIV (p=0.002), and anal sex (p=0.004). Homosexual youth (odds ratio-OR=13.46; CI95%;1.14-15.84), with a perception of high risk of acquiring HIV (OR=18.11; CI95%: 2.28-143.69) had a higher chance of being HIV positive. Conclusion the prevalence of HIV and associated factors in youth was ascertained. Contributions to practice: the information produced here shows evidence for those who aim to formulate policies, including health managers and workers. It can give support to the elaboration of strategies that involve perfecting preventive, coordinated actions, adjusted to behavioral contexts in the settings studied.

Resumen Se realizó un estudio descriptivo observacional transversal en los municipios de Posadas y Garupá con el objetivo de describir la situación social, el conocimiento sobre infecciones de transmisión sexual (ITS) y la prevalencia de infección por Treponema pallidum y por el virus de inmunodeficiencia humana (HIV) en adolescentes escolarizados en área vulnerables del Departamento Capital de Misiones (Argentina). Se realizaron 1336 encuestas y 347 pruebas serológicas a adolescentes de 19 establecimientos de nivel primario y secundario, el 64% (857) fueron de Posadas y el 36% (479) de Garupá. La mediana de la edad fue de 13 años. Las respuestas afirmativas sobre la información suministrada en salud reproductiva fue en Posadas 73% y en Garupá 49%; el ámbito de información citado con mayor frecuencia fueron las escuelas (Posadas 92% y Garupá 76%). La mayoría respondió que en su grupo de amigos se ingerían bebidas con alcohol (Posadas 64% y Garupá 52%) y, con menor frecuencia, drogas (Posadas 21% y Garupá 17%). Las respuestas correctas sobre conocimientos de ITS fueron superiores al 82%. Más del 80% residían en casas de material con baño instalado y el 9% en condiciones de hacinamiento. No se identificaron adolescentes escolarizados con infección por HIV, pero sí un caso con infección por T. pallidum (prevalencia: 0,29%; intervalo de confianza del 95%: 0,05-1,62%). El estudio mostró muy baja prevalencia de infección por T. pallidum y nula para HIV en adolescentes escolarizados residentes en áreas socialmente vulnerables. La constitución familiar y la escolarización podrían constituir factores de protección para estas infecciones.


Abstract A cross-sectional observational descriptive study was carried out in the municipalities of Posadas and Garupá with the aim of describing the social situation, knowledge about sexually transmitted infections (STIs) and the prevalence of Treponema pallidum and human immunodeficiency virus (HIV) infection in adolescents enrolled in vulnerable areas of the Capital Department of Misiones (Argentina). A total of 1,336 surveys and 347 serological tests were carried out on adolescents from 19 primary and secondary schools, 64% (857) were from Posadas and 36% (479) from Garupá. The median age was 13 years. The affirmative answers about the information provided in reproductive health were 73% in Posadas and 49%; in Garupá; the area of information most frequently cited was schools (Posadas 92% and Garupá 76%). Most responded that their group of friends drank alcoholic beverages (Posadas 64% and Garupá 52%) and, less frequently, drugs (Posadas 21% and Garupá 17%). The correct answers about knowledge of STIs were higher than 82%. More than 80% resided in brick houses with installed bathrooms and 9% in overcrowded conditions. School adolescents with HIV infection were not identified, but one case with T. pallidum infection was detected (prevalence: 0.29%; 95% confidence interval: 0.05-1.62%). The study showed a very low prevalence of T. pallidum infection and zero prevalence for HIV in school-going adolescents residing in socially vulnerable areas. Family constitution and schooling could be protective factors for these infections.


Resumo Foi realizado um estudo descritivo observacional transversal nos municípios de Posadas e Garupá com o objetivo de descrever a situação social, o conhecimento sobre infecções sexualmente transmissíveis (ISTs) e a prevalência da infecção pelo Treponema pallidum e pelo vírus da imunodeficiência humana (HIV) em adolescentes matriculados em áreas vulneráveis do Departamento Capital de Misiones (Argentina). Foram realizadas 1.336 inquéritos e 347 testes sorológicos em adolescentes de 19 escolas de ensino fundamental e médio, sendo 64% (857) de Posadas e 36% (479) de Garupá. A mediana de idade foi de 13 anos. As respostas afirmativas sobre as informações prestadas em saúde reprodutiva foram em Posadas 73% e em Garupá 49%; a área de informação mais citada foi as escolas (Posadas 92% e Garupá 76%). A maioria respondeu que seu grupo de amigos consumia bebidas alcoólicas (Posadas 64% e Garupá 52%) e, com menor frequência, drogas (Posadas 21% e Garupá 17%). As respostas corretas sobre o conhecimento de ISTs foram superiores a 82%. Mais de 80% residiam em casas materiais com banheiros instalados e 9% em condições de superlotação. Adolescentes escolares com infecção pelo HIV não foram identificados, mas foi identificado um caso com infecção por T. pallidum (prevalência: 0,29%; intervalo de confiança de 95%: 0,05-1,62%). O estudo mostrou prevalência muito baixa de infecção por T. pallidum e prevalência zero para HIV em adolescentes escolares residentes em áreas de vulnerabilidade social. A constituição familiar e a escolaridade podem ser fatores protetores para essas infecções.

Resumen Introducción: En la última década, millones de venezolanos han emigrado de su país; Colombia ha sido su principal destino. Es importante evaluar las repercusiones de la migración sobre eventos de interés en salud pública como la infección por el VIH, a fin de adoptar políticas pertinentes para atender la epidemia. Sin embargo, la información del estado de salud de la población migrante y su impacto sobre la salud pública en Colombia es escasa. Objetivos: Evaluar el papel de los casos de VIH importados desde Venezuela en el comportamiento del evento VIH/SIDA/ mortalidad por VIH en Colombia. Metodología: Mediante cálculo integral se compara el área bajo las curvas de casos de VIH totales e importados notificados al Sistema Nacional de Vigilancia en Salud Pública de Colombia. Resultados: La curva de casos de VIH importados desde Venezuela tiene una contribución menor (2,26 %) en el incremento de casos notificados en Colombia. Conclusiones: La migración venezolana no es el principal factor responsable del incremento de casos de VIH en Colombia, deben evaluarse otros factores involucrados en la difusión de la pandemia a nivel local.


Abstract Introduction: Millions of Venezuelans have emigrated due to their country's socio-economic crisis in the last decade; Colombia has been their main destination. To adopt relevant policies to address the problem, it is important to assess the impact of migration on public health interest events such as HIV infection. However, information on the health status of immigrants and its repercussions on public health in Colombia is scarce. Objective: Outline how the Venezuelan migration phenomenon has impacted HIV notification within Colombia. Methodology: Through an integral calculation, the area under the curves of total and imported HIV cases reported to the National Public Health Surveillance System of Colombia are compared. Incidence measures are carried out among the migrant population and the Colombian regions with the highest concentration of migrants are analyzed in greater depth. Results: The curve of HIV cases imported from Venezuela has a smaller contribution (2.26%) in the increase of cases notified in Colombia. Conclusions: The migratory crisis is not the main cause responsible for the increase in cases in Colombia. Other factors involved in the spread of the pandemic at the local level must be evaluated.

OBJECTIVE: Sexual minority men (SMM) of colour are disproportionately impacted by HIV and bacterial STIs (bSTIs). To better understand within-group heterogeneity and differential risk factors by race and ethnicity, we sought to examine rates of undiagnosed HIV and rectal bSTI at the intersection of racial and ethnic identity with other sociodemographic factors. METHODS: We examined data from 8105 SMM conducting home-based self-testing at enrolment in a nationwide cohort study collected from November 2017 to August 2018. We conducted analyses stratified by racial and ethnic groups to examine within-group (ie, subgroup) unadjusted rates of HIV and rectal bSTI infection across a range of characteristics. RESULTS: Rates of undiagnosed HIV were highest among Black (4.3%, n=39) and Latino (2.4%, n=38) SMM, with lower rates among those identified as multiracial (1.6%, n=15), white (1.3%, n=56) and other races (1.3%, n=6). Across the stratified analyses of HIV infection, 15 significant associations emerged showing that age, region, insurance type, sexual positioning and incarceration history had differential impacts across racial and ethnic groups. In particular, private and public insurance were protective against HIV for white but not Black and Latino SMM, and incarceration was associated with substantially higher rates of HIV infection for Black and Latino SMM relative to white SMM. We found significant co-occurrence of HIV and bSTI rates for participants who identified as Latino (OR=7.5, 95% CI 2.12 to 26.54), white (OR=3.19, 95% CI 1.14 to 8.98) and multiracial (OR=5.5, 95% CI 1.08 to 27.90), but not those who identified as Black (OR=0.82, 95% CI 0.10 to 6.56) or other races (OR=3.56 95% CI 0.31 to 40.80). CONCLUSIONS: Stratified analyses showed differential rates of HIV infection at the intersection of racial and ethnic groups with other characteristics, particularly insurance status and incarceration history, pointing to structural inequities rather than individual behaviours underlying disproportionately high rates of HIV for Black and Latino SMM.

Resumo Na ocorrência de resultado positivo para HIV em triagem sorológica para doação, o serviço de hemoterapia deve, além de descartar a bolsa de sangue, encaminhar o doador ao serviço de referência. A situação, no entanto, traz dilemas éticos implícitos, vivenciados cotidianamente pelos profissionais da área. Assim, o objetivo do presente estudo é revisar pormenorizadamente a legislação sobre o assunto, desenvolvendo reflexões necessárias. Tendo em vista que a condição de ser portador de HIV/aids está envolta em preconceitos, discriminações e repercussões sociais negativas, é fundamental que o profissional da hemoterapia responsável por comunicar a inaptidão esteja ciente do sigilo e da confidencialidade das informações e devidamente capacitado para atuar nessa situação. O artigo defende a comunicação plena da inaptidão sorológica ao doador.


Abstract A recent article published in this journal on confidentiality in the care of HIV/aids patients reports that, in the event of a positive result in the Screening Tests for the virus, the Blood Bank must, in addition to disposing of the blood bag, refer the donor to the reference service. Due to the implicit ethical dilemmas we have experienced, this led us to a detailed review of the respective brazilian legislation and critical analysis of the issue. People with HIV/aids are surrounded by prejudices, discrimination and negative social repercussions, so it is essential that the Blood Bank professional who will communicate this news is fully aware of the issues involving secrecy and confidentiality, and fully trained and capable to proceed adequately. We present and discuss the fundamental points of the current Brazilian legislation the theme and how we must communicate to the donor about the detection of HIV serodiagnosis.


Resumen En caso de resultado positivo para VIH en cribado serológico para donación, el servicio de hemoterapia debe desechar la bolsa de sangre y remeter el donante a un servicio de salud de referencia. Esta situación, sin embargo, conlleva dilemas éticos implícitos que los profesionales del ramo enfrentan cada día. Así, el objetivo de este estudio es revisar en detalle la legislación sobre la materia, desarrollando reflexiones necesarias. Tengo en cuenta que la condición de ser portador de VIH/sida conlleva prejuicios, discriminación y repercusiones sociales negativas, es fundamental que el profesional de hemoterapia encargado de comunicar la inaptitud sea consciente del secreto y de la confidencialidad de la información y también debidamente capacitado para actuar en esta situación. Este artículo defiende la plena comunicación de inaptitud serológica al donante.

BACKGROUND: The assessment of geographical heterogeneity of HIV among men who have sex with men (MSM) and people who inject drugs (PWID) can usefully inform targeted HIV prevention and care strategies. OBJECTIVE: We aimed to measure HIV seroprevalence and identify hotspots of HIV infection among MSM and PWID in Nigeria. METHODS: We included all MSM and PWID accessing HIV testing services across 7 prioritized states (Lagos, Nasarawa, Akwa Ibom, Cross Rivers, Rivers, Benue, and the Federal Capital Territory) in 3 geographic regions (North Central, South South, and South West) between October 1, 2016, and September 30, 2017. We extracted data from national testing registers, georeferenced all HIV test results aggregated at the local government area level, and calculated HIV seroprevalence. We calculated and compared HIV seroprevalence from our study to the 2014 integrated biological and behavioural surveillance survey and used global spatial autocorrelation and hotspot analysis to highlight patterns of HIV infection and identify areas of significant clustering of HIV cases. RESULTS: MSM and PWID had HIV seroprevalence rates of 12.14% (3209/26,423) and 11.88% (1126/9474), respectively. Global spatial autocorrelation Moran I statistics revealed a clustered distribution of HIV infection among MSM and PWID with a <5% and <1% likelihood that this clustered pattern could be due to chance, respectively. Significant clusters of HIV infection (Getis-Ord-Gi* statistics) confined to the North Central and South South regions were identified among MSM and PWID. Compared to the 2014 integrated biological and behavioural surveillance survey, our results suggest an increased HIV seroprevalence among PWID and a substantial decrease among MSM. CONCLUSIONS: This study identified geographical areas to prioritize for control of HIV infection among MSM and PWID, thus demonstrating that geographical information system technology is a useful tool to inform public health planning for interventions targeting epidemic control of HIV infection.

BACKGROUND: Given the rampant HIV epidemic among men who have sex with men (MSM) in Chengdu, southwest China, Treat All policy, defined as immediate antiretroviral therapy (ART) initiation after HIV diagnosis, was implemented since 2014. Real-world research evaluating impacts of immediate ART on HIV epidemics is needed to optimize policy-making as national and international guidelines have been lowering ART eligibility threshold. The purpose of this study is to: assess temporal trends of the HIV epidemic and impacts of Treat All policy among MSM; and lay foundation for HIV-related policy evaluation using longitudinal routine data from health information systems. METHODS: Data used in this study were HIV sentinel seroprevalence, annual reported HIV cases and ART coverage rate among MSM in Chengdu from 2008 to 2018, derived from national HIV/AIDS information system. Temporal trends of the HIV epidemic were described using Joinpoint Regression Program. Interrupted time-series method was deployed to evaluate Treat All policy. RESULTS: HIV sentinel seroprevalence rose from 11.20% in 2008 to 17.67% in 2013 and Annual Percent Change (APC) was 8.25% (95% CI - 2.40%, 20.07%), then decreased to 5.17% in 2018 (APC = - 19.63%, 95% CI - 27.54%, - 10.86%). Newly reported HIV cases increased from 168 cases in 2008 to 1232 cases in 2015 (APC = 26.99%, 95% CI 21.32%, 32.93%), and reduced to 1014 cases in 2018 (APC = - 8.80%, 95% CI - 18.45%, 2.01%). ART coverage rate has been climbing from 11.11% in 2008 to 92.29% in 2018 and Average Annual Percent Change was 16.09% (95% CI 11.76%, 20.59%). Results of interrupted time-series models showed that compared to an annual increase of 0.87% during pre-policy period, there was a decline of 3.08% (95% CI - 0.0366%, - 0.0250%) per year of HIV sentinel seroprevalence since 2014; and compared to an annual increase of 116 cases before 2014, there was an annual drop of 158 newly reported HIV cases (95% CI - 194.87%, - 121.69%) during the post-policy period. CONCLUSIONS: Immediate ART after HIV diagnosis could potentially curb HIV transmission at population level among MSM, along with other strategies. Future assessment of HIV prevention and control policy can be carried out using routinely collected longitudinal data from health information systems.

Objective: to evaluate factors associated with inadequate control of viral load in individuals with Human Immunodeficiency Virus (HIV) in the The Center for Assistance to Sexually Transmitted Infections/SIDA/Viral Hepatitis (CAP) of Itajubá, MG, Brazil. The acquired immunodeficiency syndrome remains a health challenge in Brazil. Therapeutic failures, characterized by detectable viral load, must have their causes evaluated. Among the most relevant reasons is the lack of adherence to treatment. Materials and methods: this is an observational, cross-sectional and documentary study of 261 medical records. The variables analyzed were socio-epidemiological characteristics and laboratory tests for viral load and CD4+ T lymphocytes, poor adherence in history or currently, duration of antiretroviral therapy (ART) use, depression and/or anxiety, use of illicit drugs, follow-up time at the CAP. Results: among the patients, 90.42% had an undetectable viral load and 64.37% had a CD4+ T count ≥500 in the last available test. Some characteristics were related to detectable viral load in the last exam: history of poor adherence during treatment (p<0,0001), inconsistent use of ART (p<0,0001) and use of illicit drugs (p=0,0155). Anxiety and/or depression were not statistically significant (p=0,3321). Conclusion: history of poor adherence, inconsistent use of ART and use of illicit drugs were associated with an increased risk of virologic failure. Early identification of groups at risk of poor adherence to treatment can support the development of intervention strategies in an transdisciplinary way to improve adherence and generate better results in the control of HIV infection..Au


Objetivo: evaluar los factores asociados con el control inadecuado de la carga viral en individuos con virus de inmunodeficiencia humana (VIH) en el Centro de Asistencia para Infecciones de Transmisión Sexual/SIDA/Hepatitis Viral (CAP) de Itajubá, MG, Brazil. El síndrome de inmunodeficiencia adquirida sigue siendo un desafío para la salud en Brasil. Las fallas terapéuticas, caracterizadas por una carga viral detectable, deben tener sus causas evaluadas. Entre las razones más relevantes está la falta de adherencia al tratamiento. Materiales y métodos: estudio observacional, transversal y documental con 261 registros médicos. Las variables analizadas fueron características socioepidemiológicas y pruebas de laboratorio para carga viral y linfocitos T CD4+, pobre adherencia en la historia o en la actualidad, duración del uso de terapia antirretroviral (ARTE), depresión. y/o ansiedad, uso de drogas ilícitas, tiempo de seguimiento en el CAP. Resultados: de los pacientes, el 90.42% tenía una carga viral indetectable y el 64.37% tenía un recuento de CD4 + T ≥500 en la última prueba disponible. Fueron evidenciadas características relacionadas con la carga viral detectable en el último examen: antecedentes de mala adherencia durante el tratamiento (p <0,0001), uso inestable de ARTE (p <0,0001) y uso de drogas ilícitas (p = 0 , 0155). La ansiedad y / o depresión no fue estadísticamente significativa (p = 0.3321). Conclusión: el historial de adherencia deficiente, el uso inconsistente de ART y el uso de drogas ilícitas se asociaron con un mayor riesgo de falla virológica. La identificación de grupos en riesgo de mala adherencia al tratamiento puede ayudar a desarrollar estrategias de intervención de manera temprana y entre disciplinas para mejorar la adherencia y generar mejores resultados en el control de la infección por VIH..Au

Objetivo: descrever os aspectos epidemiológicos da morbimortalidade pelo Vírus da Imunodeficiência Humana no nordeste brasileiro entre 2013 e 2017. Métodos: estudo descritivo e transversal realizado com dados do Sistema de Informações Hospitalares. Selecionou-se as variáveis: unidades federativas, sexo, faixa etária, e cor/raça. Resultados: registrou-se 34.647 internações no nordeste brasileiro. Destes, 4.031 evoluíram para o óbito, correspondendo a mortalidade de 11,63%. Evidenciou-se maior morbidade (30,77%) e óbito (19,40%) em Pernambuco e maior mortalidade em Alagoas (19,40%). Os homens prevaleceram nas internações (65,98%), óbitos (69,51%) e mortalidade (12,26%). Os adultos entre 35 e 39 anos tiveram maior morbidade (17,66%) e óbitos (17,39%), já os idosos ≥80 anos, tiveram maior mortalidade (22,64%). A raça/cor parda prevaleceu nas internações (51,36%) e óbitos (49,71%) e os indígenas na mortalidade (50%). Conclusão: a epidemiologia das infecções reflete na necessidade de intervenções em saúde principalmente no estado de Alagoas por apresentar maior mortalidade pela patologia

Results: it was registered 34.647 hospitalizations in Brazilian northeast. Of these, 4.031 died, corresponding to the mortality of 11,63%. It were evidenced higher morbidity (30,77%) and death (19,40%) in Pernambuco and higher mortality in Alagoas (19,40%). The men have prevailed in the hospitalizations (65,98%), deaths (69,51%) and mortality (12,26%). The adults between 35 and 39 years old had greater morbidity (17,66%) and deaths (17,39%) and the elderly ≥80 years old had greater mortality (22,64%). The brown color/race have prevailed in the hospitalizations (51,36%) and deaths (49,71%) and the indigenous in the mortality (50%). Conclusion: the epidemiology of infections reflects in the need of health care interventions, mainly in the state of Alagoas for presenting greater mortality due pathology

RESUMO Objetivo: Analisar os fatores sociodemográficos e comportamentais associados à positividade ao Vírus da Imunodeficiência Humana (HIV) em usuários de um Centro de Testagem e Aconselhamento (CTA). Método: Estudo transversal com 5.229 usuários que realizaram o teste rápido para HIV, registrados no Sistema de Informação do CTA. As análises bivariadas e multivariadas foram realizadas utilizando-se a regressão logística binária, com apresentação do OddsRatio, intervalo de confiança de 95% e p-valor <0,05. Resultados: A prevalência de infecção pelo HIV foi de 5,0% (259), com maior acometimento da população mais jovem (p=0,010). Observou-se maior positividade entre as pessoas vivendo com HIV/Aids (91,3%; p<0,001) e homens que fazem sexo com homens (HSH) (20%; p<0,001). Nas análises multivariadas verificou-se maior associação à infecção pelo HIV no modelo 2 que inclui, as variáveis sociodemográficas e comportamentais, como: o recorte populacional de HSH, o compartilhamento de seringas, a orientação sexual HSH, ter infecções sexualmente transmissíveis (IST) nos últimos 12 meses, parceiro soropositivo para HIV e uso irregular ou não uso do preservativo nos últimos 12 meses com parceiro fixo. Conclusão: A vulnerabilidade ao HIV foi mais associada aos fatores relacionados ao compartilhamento de seringas e ao comportamento sexual, especialmente os HSH e as parcerias fixas.

En los análisis multivariados se verificó mayor asociacióna la infección por el VIH en el modelo 2 que incluye las variables sociodemográficas y comportamentales como: el recorte poblacional de HSH, el compartir jeringas, la orientación sexual HSH, tener enfermedades de transmisión sexual (ETS) en los últimos 12 meses, compañero seropositivo para VIHy uso irregular o la falta del uso de preservativo en los últimos 12 meses con compañero fijo. Conclusión: la vulnerabilidad al VIH fue más asociada a los factores relacionados al compartir jeringas y al comportamiento sexual, especialmente los HSH y los compañeros fijos.


ABSTRACT Objective: To analyze the sociodemographic and behavioral factors associated with Human Immunodeficiency Virus (HIV) positivity in users of a Counseling and Testing Center (CTC). Method: Across-sectional study with 5,229 users who performed the rapid HIV test, registered in the CTC's Information System. Bivariate and multivariate analyzes were performed using binary logistic regression, presenting OddsRatio, 95% confidence interval and p-value <0.05. Results: The prevalence of HIV infection was 5.0% (259), with greater involvement of the younger population (p=0.010). Greater positivity was observed among people living with HIV/AIDS(91.3%; p <0.001) and men who have sex with men (MSM) (20%; p<0.001). In multivariate analyzes, there was a greater association with HIV infection in model 2, which includes sociodemographic and behavioral variables, such as: the populational cut of MSM, needle sharing, MSM sexual orientation, having sexually transmitted infections (STIs) in the last 12 months, HIV-positive partner and irregular or no condom use in the last 12 months with a steady partner. Results: Conclusion: Vulnerability to HIV was more associated with factors related to needle sharing and sexual behavior, especially MSM and steady partnerships.

BACKGROUND: Blood transfusion is a life-saving procedure; millions of lives are saved each year. However, blood transfusions are associated with certain risks that can lead to adverse consequences. This study aimed to survey the prevalence and trend of hepatitis B virus (HBV), hepatitis C virus (HCV), and Human immunodeficiency virus (HIV) among blood donors of Fars province, Iran (2006-2018). METHODS: This retrospective cross-sectional study was conducted by reviewing the records of the blood transfusion organization of Fars province. A total of 1952478 blood units were screened for transfusion-transmitted infections (TTIs). Then, data were entered into SPSS software (Negare. version 25). Chi-square test was used to compare the sof TTIs among blood donors. Chisquare test for trend was used to analyze the variations in trends of TTIs during this period. Finally, p-values less than 0.05 were considered statistically significant. GraphPad Prism software was used for the depiction of the graphs. RESULTS: Among the 1952478 blood donations within the 13-years, 4479(0.229 %) of donors were HBsAg, HCV Ab, and HIV Ag-Ab positive. The seroprevalence of HBV, HCV, and HIV was 2684(0.137%), 1703(0.087 %), and 92(0.0047%), respectively. CONCLUSION: The current study showed that the overall prevalence of TTIs among blood donors was low and had a descending trend over the years of study.

INTRODUCTION: Female sex workers (FSW) are considered a key group for HIV transmissions in sub-Saharan Africa. The HIV Care Continuum and HIV drug resistance (HIVDR) among FSW has not been well studied in most countries in West Africa. In the current study we describe the HIV Care continuum and prevalence of HIVDR among FSW in Guinea-Bissau. METHODS: A venue-based recruitment and peer-referral of FSW was used in seven cities in Guinea-Bissau from October 2014 to September 2017. We administered a questionnaire, performed discriminatory HIV-testing and collected blood specimens for CD4 count, viral load and HIVDR genotyping. RESULTS: The survey included 440 FSW. The overall HIV-prevalence among FSW was 26.8%. Of the HIV-1 (HIV-1 single- or dually HIV-1/HIV-2) infected FSW (N = 104), 58.7% were previously diagnosed with HIV-1 at enrolment and 41.4% reported taking antiretroviral therapy (ART) compared to 28.6% of the HIV-2 single-infected FSW (N = 14). Among HIV-1 infected FSW on ART (N = 43), 55.8% were virally suppressed (< 1000 copies/ml) and of all HIV-1 infected FSW, 29.8% were virally suppressed. Among ART experienced FSW (N = 22), 50.0% had HIVDR. HIVDR was also found in 9.4% of treatment naïve FSW (N = 53). CONCLUSION: The majority of FSW who knew their HIV status received ART, however a large proportion of FSW were not aware of their HIV positive status. This translated into a great majority of the HIV-infected FSW not being virally suppressed. Amongst treatment naïve FSW nearly a tenth had HIVDR, suggesting that sexual transmission of HIVDR is occurring in this at-risk-population.

ABSTRACT OBJECTIVE: To examine the HIV care cascade among trans women and travestis in São Paulo - Brazil, the most populous city in South America. METHODS: Using data from a cross-sectional study carried out between November 2016 and May 2017 in the city of São Paulo (Divas Research). Respondent driven sampling (RDS) was used to recruit 386 transgender women and travestis who participated in a HIV risk survey and were tested for HIV. The cascade was defined as HIV prevalence, HIV diagnosed, Antiretroviral (ART) Prescription, and currently on ART. A multiple analysis model was conducted to identify the association between sociodemographics and the cascade gaps. RESULTS: Of the trans women living with HIV, 80.9% were already diagnosed, 76.6% of them had been prescribed, of which 90.3% were currently on treatment. Those who were registered in care had a higher rate of ART (aPR 2.06; 95%CI 1.09-3.88). Trans women between 31-40 years old (aPR 1.65; 95%CI 1.09-2.50) and those older than 40 (aPR 1.59; 95%CI 1.04-2.43) had higher prevalence of ART. CONCLUSIONS: Our data suggest an increase in the testing and treatment policy implementation among trans women in the city of São Paulo, although gaps have been found in the linkage to care. However, young trans women and those not registered in health care service may benefit from efforts to engage this part of the population in care to improve HIV treatment and care outcomes.

BACKGROUND: Women-specific factors exist that increases vulnerability to drug-related harms from injection drug use, including blood-borne viruses (BBVs), but gender-based differences in BBV prevalence have not been systematically examined. METHODS: We conducted meta-analyses to estimate country, regional, and global prevalence of serologically confirmed human immunodeficiency virus (HIV), hepatitis C virus (HCV; based on detection of anti-HCV antibody), and hepatitis B virus (HBV; based on detection of HBV surface antigen) in people who inject drugs (PWID), by gender. Gender-based differences in the BBV prevalence (calculated as the risk among women relative to the risk among men) were regressed on country-level prevalence and inequality measures (Gender inequality index, Human development index, Gini coefficient, and high, low or middle income of the country). RESULTS: Gender-based differences varied by countries and regions. HIV prevalence was higher among women than men in sub-Saharan Africa (relative risk [RR], 2.8; 95% confidence interval [CI], 1.8-4.4) and South Asia (RR, 1.7; 95% CI, 1.1-2.7); anti-HCV was lower among women in the Middle East and North Africa (RR, 0.6; 95% CI, .5-.7) and East and Southeast Asia (RR, 0.8; 95% CI, .7-.9). Gender-based differences varied with country-levels of the BBV prevalence in the general population, human development, and income distribution. CONCLUSION: HIV was more prevalent in women who inject drugs as compared to their male counterparts in some countries, but there is variation between and within regions. In countries where women are at higher risks, there is a need to develop gender-sensitive harm-reduction services for the particularly marginalized population of women who inject drugs.

ABSTRACT OBJECTIVE To describe the epidemiological aspects of HIV infection and AIDS among indigenous peoples of the state of Mato Grosso do Sul, Brazil. METHODS This is a descriptive epidemiological study on the occurrence and distribution of HIV infection and AIDS in the indigenous population assisted by the Distrito Sanitário Especial Indígena (Indigenous Special Health District) Mato Grosso do Sul between 2001 and 2014, based on three secondary databases. Annual rates of HIV and AIDS detection and prevalence were calculated, considering case distribution according to village, Health Base Pole and sociodemographic variables. Accumulated rates of detection, mortality and case fatality were calculated by ethnic group and for the Health Base Pole with the highest number of cases. RESULTS The HIV detection rate fluctuated between 0.0 and 18.0/100 thousand people in the study period. For AIDS, there was no notification before 2007, but in 2012 its rate reached 16.6/100 thousand. HIV prevalence grew between 2001 and 2011, and it continuously grew for AIDS starting from 2007. The highest HIV detection rates occurred among Guarani peoples (167.1/100 thousand) and for AIDS, among the Kaiowá peoples (79.3/100 thousand); mortality and fatality rates were higher among the Kaiowá. Regarding the Dourados Health Base Pole, the AIDS detection rate increased, and the mortality and fatality rates decreased. CONCLUSIONS HIV infection and AIDS have been increasing among indigenous peoples, with distribution of the disease mainly in the Health Base Poles of the southern region of the state, where greater economic and social vulnerability are also observed. The endemic character of HIV and AIDS can become epidemic in some years given the existence of cases in other villages in the state. Its occurrence among the Guarani and Kaiowá populations indicates the need for expanded diagnosis, access to treatment and prevention measures.


RESUMO OBJETIVO Descrever os aspectos epidemiológicos da infecção pelo HIV e da aids entre povos indígenas do Mato Grosso do Sul. MÉTODOS Estudo epidemiológico descritivo sobre ocorrência e distribuição da infecção pelo HIV e aids na população indígena assistida pelo Distrito Sanitário Especial Indígena Mato Grosso do Sul, entre 2001 e 2014, a partir de três bases de dados secundários. Calcularam-se as taxas anuais de detecção e de prevalência de HIV e aids, com distribuição dos casos segundo aldeia, Polo Base e variáveis sociodemográficas. As taxas acumuladas de detecção, mortalidade e letalidade foram calculadas por etnia e para os Polos Base com o maior número de casos. RESULTADOS A taxa de detecção de HIV flutuou entre 0,0 e 18,0/100 mil pessoas no período. Para a aids, não houve notificação antes de 2007, mas em 2012 sua taxa chegou a 16,6/100 mil. A prevalência de HIV indicou crescimento entre 2001 e 2011, e para a aids observou-se aumento contínuo a partir de 2007. As maiores taxas de detecção de HIV ocorreram entre os Guarani (167,1/100 mil) e de aids, entre os Kaiowá (79,3/100 mil); as taxas de mortalidade e letalidade foram superiores entre os Kaiowá. Para o Polo Base de Dourados, observou-se elevação da taxa de detecção de aids e diminuição das taxas de mortalidade e letalidade. CONCLUSÕES A infecção pelo HIV e a aids mostraram-se crescentes entre povos indígenas, com distribuição da doença principalmente nos Polos Base da região sul do estado, onde observa-se também maior vulnerabilidade econômica e social. O caráter endêmico do HIV e da aids pode se tornar epidêmico em alguns anos, considerando a existência de casos em outras aldeias do estado. Sua ocorrência entre os Guarani e Kaiowá sinaliza a necessidade de ampliação do diagnóstico, do acesso ao tratamento e de medidas de prevenção.

Objetivo: analisar os fatores sociodemográficos relacionados ao não uso do preservativo nas relações sexuais e a prevalência de infecções sexualmente transmissíveis (IST) em comunidades rurais de Ouro Preto, Minas Gerais, Brasil, entre 2014 e 2016. Métodos: foram coletados dados de entrevista individual e realizados testes rápidos; as associações foram testadas pelo modelo de regressão de Poisson, com intervalo de confiança de 95% (IC95%). Resultados: foram detectados 3,8 casos/10 mil habitantes de hepatite B e sífilis, e 1,3/10 mil hab. de hepatite C; não foram detectados casos de HIV; na análise multivariada, foram encontradas maiores prevalências de não uso de preservativos entre indivíduos casados/em união estável/viúvos (RP=1,20 - IC95% 1,06;1,36). Conclusão: o grupo com maior prevalência de não uso de preservativo é o de pessoas com relacionamento fixo; novos casos de sífilis e de hepatites virais foram detectados pelo teste rápido, aplicado no inquérito.

Métodos: se recolectaron datos en entrevista individual y se realizaron pruebas rápidas; las asociaciones fueron analizadas por la regresión de Poisson, con intervalo de confianza de 95% (IC95%). Resultados: se detectaron 3,8 casos/10 mil habitantes de hepatitis B y sífilis, y 1,3/10 mil habitantes de hepatitis C; no se detectaron casos de VIH; el análisis multivariado mostró mayor prevalencia de no uso del preservativo para personas casadas /en unión estable/viudos (RP=1,20 - IC95% 1,06;1,36). Conclusión: se detectaron nuevos casos de sífilis y hepatitis virales por las pruebas rápidas en esta investigación; el grupo de mayor riesgo de no usar preservativo fue de personas casadas/con relación fija.

Results: we detected 3.8 cases/10,000 inhabitants for hepatitis B and syphilis, and 1.3 cases/10,000 inhabitants for hepatitis C; no HIV cases were detected; in the multivariate analysis we found higher prevalence rates of condom non-use among the group of individuals who were married, had common law partners or were widowed (PR=1.20 - 95%CI 1.06;1.36). Conclusion: individuals in a stable relationship formed the group with the highest prevalence rate of condom non-use; new syphilis and viral hepatitis cases were detected using rapid tests during the survey.
